GENERAL INFORMATION APPLICABLE TO ALL PASSPORT SERVICES:

All applicants should submit the application in person for identification (Including minors and new born babies) at the Embassy counter.

The registration of birth of a minor child under section 4 (1) of the citizenship act, 1955 has to be done online at http://indiancitizenshiponline.nic.in/ic_form_public.aspx . Thereafter, the duly filled form should be downloaded and submitted to the Embassy along with the prescribed form Click here.

An ordinary passport is valid for ten years from the date of issue. Passport Issuing Authority can issue passports even for a shorter duration. Minors are issued passports for 5 years or up to attaining 18 years, whichever is earlier.

The Passport service asked for should be in the proper prescribed form accompanied with appropriate fees. The application for the service in any other form is not acceptable. Applicants are advised to ensure that their application in the prescribed format is complete in all respects; that all columns are correctly and legibly filled up and no column is left blank or unfilled / un-responded or vaguely filled with a dash, tick, etc. All signatures in the application should be with ballpoint pen of black or blue ink. Those who cannot affix their signatures like infants, minors or illiterate should put their thumb impression in the place of signature.

It is an offence under the Passport Act 1967 to furnish false information in the application. Passport facilities would be denied on grounds of suppression of material information, submission of incorrect particulars, willful damage/loss of passport or for unauthorized change/tampering. The Passport can be impounded or revoked for violation of one or more of the provisions of the Passport Act.

It is an offence to hold more than one valid passport at a time.

A passport expired more than six months back should be supported by a Sworn Affidavit explaining the circumstances and the reasons for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.

In respect of applications for renewal/re-issue of passports issued by Passport Issuing Authorities (PIA) in India –personal particulars form (2 copies) is also to be submitted.

The signature of the applicant should exactly fit in the prescribed column (4.5 x 1.5 cm). Please ensure that the applicant fills the surname column and the given name column correctly.

In the permanent address (in India) column, please ensure that there is complete postal address including the PIN code legibly.

The delivery of processed documents is to be done preferably to the applicant himself/herself or to the person authorized by the applicant in writing and accompanied by the official receipt given to him/her at the time of submission.

The scale of fees for renewal and various passport related services and various other passport related services, including all kinds of miscellaneous services on passport, are prescribed by Government of India from time to time. The rate of fee in vogue is given along with the various services.

All applications to be accompanied by the photocopy of the passport, (first two page, last two page and page containing valid visa) Photographs should be in colour and against white background and should be of the size of 35 mm X 35 mm. The photograph should have a frontal view, with both ears and chin to forehead being visible. Eyes should be open. There should not be any distracting shadows on the face or on the background. Head coverings are not permitted except for religious reasons, but the facial features from bottom of chin to top of forehead and both edges of the face must be clearly shown. The expression on the face should look natural.

ORDINARY PASSPORT

RENEWAL/REISSUE OF PASSPORT ON EXPIRY OF VALIDITY /EXHAUSTION OF VISA PAGES

(NOTE: A passport can be renewed anytime within one year before its expiry date)

Requirements:

2. Five copies of recent photos with white background and frontal view. (35mmx35mm).

3. Current Passport in original with photocopy (of first 2, last 2, address page if separate page, any other endorsement pages and visa page with additional booklets, if any. For minors (up to 18 years) parents’ passport copies are required.

4. A passport expired more than 6 months ago should be supported by a Sworn Affidavit explaining the circumstances and the reasons for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.

6. In the case of passports issued by other Passport Issuing Authorities – Personal Particulars Form (in Triplicate with pasted photos).

7. Personal presence of the applicant is a must.

LOSS AND DAMAGE OF PASSPORT

NEW PASSPORT IN LIEU OF LOST PASSPORT

Requirements

2. Damaged passport, in original, with a photocopy.

3. Five copies of recent photo with white background and frontal view. (35mmx35mm).

4. Copy of the First Information Report (FIR) from the Police authorities and its authentic legal English translation (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage)

5. Original with a copy of one or more of the following photo-affixed identity cards to establish identity and nationality (The original will be returned after verification):

(a) Identity Card (b) Labour Card (c)Health Card (d) Driving License (e) Election commission card

6. Newspaper advertisement, in original, in two leading local dailies (one each in English and Farsi) as per the following format.

Please attach full sheets of the newspaper and not cuttings thereof (This is required where the passport is substantially damaged or it is damaged in a fire/mishap. This document is not required if there is only a minor damage):

“Indian Passport No........................ dated............................ issued at....................................................... and held by Mr./Ms.......................................................................s/o, d/o, w/o............................................................ has been genuinely damaged. 7. A self declaration signed by the applicant

a) The circumstances leading to the damage of the passport;

b) Details of travel on the damaged passport;

c) Places of stay and work in Iran since arrival

9. Personal appearance of applicant is compulsory to establish physical identity.

NOTE: IN THE CASE OF APPLICANTS WHO ARE ON CASUAL VISIT TO IRAN AND NOT A RESIDENT IN THIS COUNTRY, HE/SHE WILL BE ISSUED AN EMERGENCY CERTIFICATE TO RETURN TO INDIA.

CHANGE OF ADDRESS (involving issue of New Passport)

4. Documentary evidence of change of address like Nativity Certificate in original issued by District Collector /Tehsildar /Commissioner/ Panchayat or copies of ration card, Election Commission ID card, Title deed, water, electricity or telephone bill in applicant’s name,

and should be attested by the Home Department of the State Government concerned. For minor’s parent’s passport copy is accepted as a proof.

5. If the validity of the present passport expired more than 6 months ago, Sworn Affidavit for late submission, signed by applicant in the presence of Consular Officer.

6. Personal Particulars Form (in Triplicate with pasted photos) of the existing passport, if the passport is issued by other Passport Issuing Authorities.

7. Personal appearance of applicant is compulsory to establish physical identity.

Passport Will be retained during the Tenure of the Service.

POLICE CLEARANCE CERTIFICATE (PCC)

1. Application in form no 2 available at http://passport.gov.in/cpv/miscell.pdf .

2. Two recent photos with white background and frontal view. (35mmx35mm).

3. Current Passport in original with photocopy (of first 2, last 2, address page if separate page, any other endorsement pages and visa page with additional booklets if any.

4. Personal Particulars Form (in Triplicate with pasted photos) of the existing passport, if the passport is issued by other PIAs.

5. Copy of Latest Police Clearance Certificate in English (original to be shown) obtained from

Iranian Authorities.

6. Clearance from the Embassy.

7. Personal appearance of applicant is compulsory to establish physical identity.

Passport Will be retained during the Tenure of the Service.

Time Taken - On receipt of clearance from the concerned authorities in India for PCC for stay in Iran & India.

CANCELLATION OF ECR ENDORSEMENT- IT WOULD CALL FOR ISSUANCE OF NEW PASSPORT

(In case the passport booklet does not have ECR stamp, the passport holder would be deemed to have been granted ECNR status)

Requirements:

3. Current Passport in original with photocopy (of first 2, last 2, address page if separate page, any other endorsement pages and visa page with additional booklets, if any. For minors (up to 18 years) parents’ passport copies are required.

4. A passport expired more than 6 months ago should be supported by a Sworn Affidavit explaining the circumstances and the reasons for late renewal. Sworn Affidavit is to be signed by the applicant in the presence of the Consular Officer.

5. In the case of passports issued by other Passport Issuing Authorities – Personal Particulars Form (in

Triplicate with pasted photos).

6. For canceling ECR Status in the case of dependents, (children/spouse) original and a photocopy of passport of the spouse/parent.

7. Personal appearance of applicant is compulsory to establish physical identity

Passport Will be retained during the Tenure of the Service.

Who are eligible for cancellation of ECR Status:-

a. All holders of Diplomatic/official passports.

b. All Gazetted Government Servants.

c. All Income-Tax payers (including Agricultural Income-Tax Payees) in their individual capacity. Proof of assessment of Income-Tax and actual payment of Income Tax for last one year or copy of PAN card, and not merely payment of advance tax. If an assessment order is not issued separately by the Income Tax department, Income Tax Returns which are stamped by Income Tax authority can be accepted.

d. All professional Degree holders, such as Doctors holding MBBS Degree or equivalent degree in AYURVED, or Homeopathy, accredited journalists, Engineers, Chartered Accountants, Cost Accountants, Lecturers, Teachers, scientists, Advocates, etc.

e. Spouses and dependent children of category of persons listed from (b) to (d).

f. All persons having educational qualification of matriculation and above.

g. Seamen who are in possession of CDC or Sea Cadets, Deck Cadets:

h. Who have passed final examination of three year B.Sc. Nautical Sciences Course at T.S. Chanakya, Mumbai and

i. Who have undergone three months pre-sea training at any of the Government approved Training Institutes such as T.S. CHANAKYA, T.S. REHMAN, T.S. JAWAHAR, MTI(SCI) and NIPM, CHENNAI after production of Identity cards issued by the Shipping Master, MUMBAI/KOLKATA/CHENNAI.

j. Persons holding permanent Immigration Visa, such as the visas of UK, USA and Australia.

k. Persons possessing two years diploma from any Institute recognized by the National Council of Vocational Training(NCVT) or State Council of Vocational Training (SCVT) or persons holding three years’ diploma/equivalent degree from institutions like polytechnics recognized by central/state Governments.

l. Nurses possessing qualifications recognized under the Indian Nursing Council Act, 1947.

m. All persons above the age of 50 years.

n. All persons who have been staying abroad for more than three years (the period of leave /

stay in India to be deducted).

o. All children up to the age of 18 years of age. (At the time of re-issue at the age of 18 years, ECR stamping shall be done, if applicable).

NRI CERTIFICATE (FOR EDUCATIONAL PURPOSES ONLY)

1. Application in form No 2 available at http://passport.gov.in/cpv/miscell.pdf .

2. One recent photo with white background and frontal view.(35mmx35mm)

3. Valid Passport in Original (which should cover the whole previous Financial Year, i.e. from

1st April, if not previous passport also) with photocopy of first, last, address page if separate page, any other endorsement pages and visa page with additional booklets if any.

4. Passports without residence visa may also be accepted in case the Applicants had not stayed in India for more than 182 during the previous Financial Year.

5. A declaration by the applicant to the effect that he has not stayed in India for more than

182 days during the PREVIOUS Financial Year (1ST April,….. to 31st March, ……).(Format enclosed)

6. If the applicant wants to have the name of his/her son/daughter mentioned in the NRI

certificate, then the applicant should give a copy of Passport (1st and last pages) of the

child. [The names of other relative of the applicant will not be mentioned in the certificate.]

7. Personal appearance of the applicant is compulsory to establish physical identity.

DECLARATION FOR NRI CERTIFICATE / SPONSORSHIP FOR COLLEGE ADMISSION

5. Declaration Form prescribed u/s 8 of the Citizenship Act, duly attested by the Embassy of India, Tehran (An Indian citizen with passport and valid visa to accompany the applicant for signing the Declaration Form).

7. Payment of penalty, if any, to be checked by the Embassy in terms of existing relevant instructions.
